Hair Removal Release Form

I understand that hair removal (waxing/sugaring) particularly on the face, carries certain risks. Hair removal cannot be performed if using Retin A/Renova, Accutane or AHA’s. Other products and medications could cause sensitivity as well. The Risks:

  • • Redness, bruising, bleeding, hyperpigmentation on areas waxed and lifting of the skin. These conditions may be made worse using certain pharmaceuticals and cosmeceuticals, particularly those for anti-aging and anti-acne treatment.
  • • Prescription medications may aggravate the skin when waxed, particularly those causing photosensitivity. Examples are antibiotics and blood thinners.
  • • Tanning booths can also contraindicate waxing. Waxing should not be done for 24 hours before or after tanning. Any areas showing redness will also not be waxed. If you are susceptible to cold sores or herpes outbreaks, taking antiviral medication three days before your appointment is highly recommended. This is because waxing can sometimes trigger an outbreak. Acknowledgement

I understand the risks and acknowledge that minor redness, swelling, and pinpoint bleeding may be possible after a hair removal service. This is normal.

I do not use any products or medications that will cause an adverse reaction or have a medical condition that is contraindicated for a hair removal service.
